Social Services Director

The Social Services Department in accordance with current federal, state and local standards and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the residents are met/maintained on an individual basis. Social Services Director is responsible for completing and maintaining the grievance and reportable process in our facility as well as conducting all Care conferences, 72-hour meetings, MDS assessments, and individualized care plans. Social Services Director is responsible for planning, coordinating, and following through on the resident discharge process, this includes scheduling doctor discharge visits, ordering home health, PT, OT, and any DME needed for resident's safe return to their new living environment.
