Memory Care Support Manager

 Qualifications:

1.      BSW / SSD Preferred

3.      Certified Dementia Practitioner Preferred 

4.      Management Experience in Long Term Care Preferred

Job Responsibilities

1.      Complete MDS and other needed assessments

2.      Manage staff and day to day operations on memory care unit

3.      Run care plan meetings for residents on memory care unit

4.      Ensure daily programming is taking place

5.      Assist with daily programming when needed

6.      Work with other department leaders as needed to ensure smooth operation of the memory care unit.

7.      Educate staff on dementia unit related to dementia and programming

8.      As well as other duties that arise
