Responsibilities include:
Assess nature of patients' situations by interviewing and reviewing personal history
Develop and execute individual treatment plans/care plans
Determine appropriate milestones to gauge patient progress
Communicate patient updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs as needed
Maintain accurate documentation

Qualifications:
Previous experience in social work, with a focus on short term and long-term care patients
Compassionate and caring demeanor
Ability to build rapport with staff, patients, and their loved ones
Strong leadership qualities
Excellent written and verbal communication skills
