Job Summary:

Responsibilities:

• Coordinate all aspects of MDS documentation, including assessments, care plans, and quality measures, to ensure compliance with federal and state regulations.

• Collaborate with nursing staff, therapists, and other healthcare professionals to ensure accurate and complete MDS data.

• Develop and implement process improvements to optimize MDS data accuracy, reduce errors, and increase efficiency.

• Maintain accurate and detailed records of MDS assessments, care plans, and quality measures.

Requirements:
