🏠 Travel Occupational Therapist (OT) – Home Health

📍 Location: Hermiston, Oregon
📅 Contract: 13 Weeks
☀️ Schedule: Days | Monday–Friday, 8:00 AM–4:30 PM | Weekend Rotation
⏱️ Hours: 40 Hours/Week

We're seeking an experienced Occupational Therapist (OT) for a 13-week Home Health travel assignment. The OT will provide skilled therapy services in patients' homes, helping them regain independence and achieve their optimal level of function in daily activities.

✅ Requirements
Active Oregon Occupational Therapist License
Minimum 3 years of Occupational Therapy experience
Home Health Occupational Therapy experience
Graduate of an accredited Occupational Therapy program
Current BLS Certification
Valid Driver's License
Reliable personal vehicle for patient visits
Current Auto Insurance
Strong knowledge of rehabilitation and occupational therapy procedures
Ability to work independently and collaboratively with an interdisciplinary team

🏠 Clinical Experience
Evaluate patients' functional status and implement physician-directed plans of care
Complete initial assessments and reassessments using objective tests and measurements
Develop and provide individualized occupational therapy interventions
Help patients improve independence with self-care, leisure, work, and daily activities
Provide patient and caregiver education and direction
Monitor patient progress and adapt treatment plans as needed
Recognize changes in patient condition and respond appropriately to emergency situations
Communicate observations, treatment recommendations, and patient progress with the interdisciplinary team
Participate in team meetings and coordinate the plan of care
Maintain accurate and timely clinical documentation
Protect patient rights and provide compassionate, patient-centered care
