### **Job Requirements**
* Active California PT License
* Minimum 3 year of PT experience
* BLS/BCLS certification
* Valid Driver’s License, Auto Insurance & Registration
* Required immunizations and annual TB/Flu
* Willingness to travel outside assigned territory
* Comfortable seeing COVID+ and PUI patients

### **Additional Details**
* Home Health Physical Therapy
* Minimum productivity: 27 points/week
* Per-visit payment; approved hours include travel, documentation, and phone time
* Mileage reimbursement: **$0.585/mile (patient-to-patient only)**
* Holiday coverage required
* W-2 only | No 1099
* Overtime after 40 hours/week
* All RTO must be disclosed at submission
