Hospice RN ($5000 Sign on Bonus)

The Hospice RN plays a vital role in providing compassionate, high-quality nursing care to patients facing life-limiting illnesses. This position involves collaborating with interdisciplinary teams to develop and implement individualized care plans, ensuring comfort, dignity, and support for patients and their families during the end-of-life journey. The Hospice RN is committed to delivering skilled clinical care while offering emotional support and education.

Responsibilities

• Assess and monitor patient condition and symptoms regularly
• Develop and implement personalized care plans in collaboration with the care team
• Provide education and support to patients and families regarding hospice care and disease progression
• Administer medications and treatments according to hospice protocols
• Coordinate with physicians, social workers, and other healthcare professionals
• Document patient care accurately and timely
• Provide emotional and spiritual support to patients and families
• Participate in interdisciplinary team meetings to review and update care plans
