Star Light Home Health in Montrose, CO is looking for a passionate and driven Assistant Director of Nursing to join our dedicated care team. Responsibilities:

• Assist the Director of Nursing in overseeing daily clinical operations and ensuring high-quality patient care
• Supervise, mentor, and support nursing staff and field clinicians to promote best practices
• Conduct patient assessments, care plan reviews, and ensure compliance with home health regulations
• Monitor and evaluate clinical outcomes, identifying areas for quality improvement
• Ensure adherence to Medicare, Medicaid, and state/federal home health guidelines
• Participate in hiring, onboarding, and ongoing education of nursing personnel
• Collaborate with physicians, therapists, and interdisciplinary team members on patient care coordination

Requirements:

• Active RN license in the state of Colorado (or compact license eligibility)
• Minimum 1-2  years of clinical nursing experience, preferably in home health or community care
• Knowledge of Medicare/Medicaid home health conditions of participation
• Excellent communication, organizational, and critical thinking skills
• Experience with OASIS documentation and home health EMR systems a plus
• Compassionate, team-oriented leader with a commitment to patient-centered care
• Home health experience is preferred but not required. We are willing to train the right candidate who demonstrates strong clinical judgment, reliability, leadership potential, and a willingness to learn home health regulations, OASIS, documentation requirements, and agency processes.
• Must be receptive to ongoing education, training, and professional development as part of the role.
