Employment Type: Full-Time (40 hours/week)

Family Health Care is seeking a compassionate and skilled Behavioral Health & Substance Use Disorder Therapist to join our integrated healthcare team in Big Rapids, Michigan.

This full-time position provides outpatient mental health therapy, substance use disorder treatment, care coordination, and behavioral health interventions within a collaborative primary care setting. Working alongside primary care providers, MAT prescribers, and behavioral health professionals, the therapist will support individuals experiencing mental health conditions, substance use disorders, and co-occurring behavioral health needs through evidence-based, recovery-oriented care.

What You'll Do

• Provide outpatient behavioral health and substance use disorder services within an integrated primary care setting.
• Conduct comprehensive behavioral health and substance use disorder assessments, screenings, diagnoses, and treatment planning.
• Administer SBIRT screenings and deliver brief interventions.
• Provide individual therapy, crisis intervention, behavioral health consultation, and care coordination services.
• Utilize evidence-based practices including Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), relapse prevention, and trauma-informed approaches.
• Facilitate structured substance use disorder treatment groups and psychoeducational groups.
• Support patient engagement, treatment adherence, recovery goals, and relapse-prevention planning.
• Collaborate with primary care providers, MAT prescribers, community agencies, and recovery support services.
• Coordinate referrals, follow-up care, and transitions between services.
• Participate in quality improvement initiatives and population health activities.
• Maintain timely, accurate, and compliant documentation within the electronic medical record (EHR).

Qualifications Required

• Master's degree in:
  • Social Work
  • Counseling
  • Marriage and Family Therapy
• Current Michigan licensure or eligibility for licensure as:
  • LMSW or LLMSW
  • LPC or LLPC
  • LMFT
• Strong communication, engagement, crisis intervention, and organizational skills.
• Commitment to collaborative, patient-centered care.

Preferred

• Experience treating substance use disorders, co-occurring disorders, or dual-diagnosis populations.
• Experience in integrated behavioral health, primary care behavioral health, outpatient therapy, community mental health, or substance use disorder treatment settings.
• Training or experience in Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), SBIRT, trauma-informed care, and recovery-oriented practices.
• Experience facilitating therapeutic or psychoeducational groups.
• Familiarity collaborating with MAT prescribers and supporting patients receiving medication-assisted treatment.
