Job Description
About the Role
Are you someone who loves making people’s day, bringing order to chaos, and connecting with your local community? We’re looking for an energetic, organized, and heart-driven Care Manager to join our non-medical in-home care family!
In this role, you won't be stuck behind a desk all day crunching numbers. You’ll be the heartbeat of our team—matching incredible caregivers with wonderful clients, keeping operations humming, and spreading the word about the work we do.
What You’ll Be Doing
- Master of the Puzzle (Scheduling): Create seamless weekly schedules that make both our clients and caregivers say, "This is perfect!" You're the go-to problem solver who keeps everything running smoothly.
- Client & Caregiver Happiness: Conduct check-ins, build deep relationships, and ensure every care plan delivers top-notch warmth and reliability.
- Community Connector (Mini-Marketing): Spend a bit of your time out and about! Build relationships with local senior centers, healthcare providers, and community groups to share how we help families thrive.
- Team Hype Leader: Assist with welcoming and onboarding new caregivers, keeping team morale high and supportive.
Who You Are
- A People Person: You make friends easily, listen with empathy, and know how to brighten someone's room just by walking in.
- An Organizational Whiz: Tetris has nothing on you. You can juggle shifting schedules, last-minute changes, and details without losing your cool.
- Self-Motivated & Creative: You're excited to brainstorm fun community events or outreach ideas to get our name out there.
- Passionate About Care: You genuinely care about seniors and families in our community and want your daily work to have real meaning.
Why You’ll Love Working With Us
- Real Impact Every Single Day: No corporate fluff—your work directly improves lives in our community.
- Vibrant & Supportive Culture: We work hard, celebrate wins together, and treat our team like family.
- Variety: No two days are the same! One morning you're building schedules; the next afternoon you're out meeting local community partners.
- Growth & Perks: Competitive pay, flexible PTO, and room to grow as we expand!
Qualifications
- Required Education: Bachelor's Degree in Business or Healthcare Related Field; Master's Degree Preferred.
- Experience: Previous relationship building/maintaining experience with skilled nursing facilities, rehab facilities, physician offices, hospitals, hospice, home health, etc.
- EHR: Previous experience using WellSky or similar in-home care EHR.
- Marketing: Marketing Experience Required. Attend Marketing events and create opportunities to attend and/or host community and healthcare related events.
