POSITION OVERVIEW:
Community Care Connector is a full-time, non-clinical community health worker responsible for helping people navigating their reproductive and sexual health, substance use, and may be living at the intersections of one or more additional social or health disparities (i.e. food insecurity, intimate partner violence, housing instability, and poverty). This role specializes in intervention and prevention across the harm reduction spectrum.
Community Care Connector is a full-time, non-clinical community health worker responsible for helping people navigating their reproductive and sexual health, substance use, and may be living at the intersections of one or more additional social or health disparities (i.e. food insecurity, intimate partner violence, housing instability, and poverty). This role specializes in intervention and prevention across the harm reduction spectrum.
The position is grounded in SisterReach’s reproductive and sexual justice framework and uses trauma-informed, non-judgmental, harm-reduction, culturally responsive, and trans-affirming practices. The Care Connector meets participants where they are - in community settings, through mobile and telehealth engagement, and through trusted community networks - while protecting privacy, autonomy, dignity, and informed choice. This position requires strong organizational skills, cultural humility, and the ability to provide compassionate, inclusive service to clients from diverse backgrounds.
PERFORMANCE EXPECTATIONS
The Community Care Connector contributes directly to Pleasure Pak, CareOnHand and Pearl’s Pantry programs. This role also may provide additional assistance to our YouMeHIV program. This includes timely outreach to people identified as out of care; re-engagement into HIV medical care; rapid appointment scheduling; continued medical visit attendance; documented SDOH referrals; retention follow-up; HIV testing; and prevention linkages. Individual performance expectations will be based on workplans, client engagement, program implementation, and ability to navigate programmatic shifts and expectations as informed by the political climate and social environment.
RESPONSIBILITIES:
Programmatic and Community Engagement:
- Collaborate with the Director of Programs and Leadership Development, Data, Monitoring and Evaluation Contractor, clinical partners, public health partners, and community organizations to support coordinated care and program goals.
- Lead community outreach and advocacy initiatives
Pearl’s Pantry Mobile Outreach:
- Assist with Pearl’s Pantry and Pearl’s Pantry Mobile programming, including seasonal inventory rotation, in-person and virtual grocery shopping, and other related tasks.
- Support mobile outreach efforts, including client order processing, voter registration, HIV testing, syringe service and other harm reduction kit distribution and exchange, and tabling activities.
HIV Testing and Care Connection:
- Provide education, resources, and care connection for individuals seeking HIV testing, PrEP, PEP, DoxyPEP, syringe services and harm reduction kits.
- Provide free in-office and outreach-based testing for HIV and Hep C (training and certification provided).
- Connect clients to appropriate care if they test positive for HIV.
Training and Certification:
- Maintain certifications in HIV testing, care connection, and harm reduction strategies for people who use drugs (PWUD).
- Deliver services rooted in equity, respect, and cultural competency.
ADMINISTRATIVE FUNCTION:
- Execute documentation for funder requirements
- Submits weekly and quarterly work plans, including narratives summarizing activities and outcomes.
- Supports inventory monitoring and other relevant tasks assigned.
- Participates in individual and group check-ins, staff meetings, professional development, and special organizational events.

- Proficiency in Microsoft Office Suite, web-based email, and scheduling tools such as Outlook Calendar
- Proficiency in written and public communication
QUALIFICATIONS:
- Associate or bachelor’s degree in social work, public health, human services, community health, behavioral health, or a related field preferred. Relevant lived and community-based experience may substitute for formal education where appropriate.
- Experience in community health, HIV services, care navigation, outreach, harm reduction, peer support, case management, social services, or a closely related area.
- Experience working in the reproductive health, rights, justice, LGBTQIA+ or gender field
- Demonstrated ability to engage people who may have experienced stigma, discrimination, healthcare mistrust, poverty, housing instability, substance use, or other barriers to care.
- Reliable transportation with minimum liability insurance coverage and clear driving history.
- Ability to work occasional evenings/weekends for outreach and community events
- Availability to travel up to 20% of the time for meetings, training, and professional development
PREFERRED EXPERIENCE & COMPETENCIES
- Lived experience with HIV, substance use disorder, intimate partner violence and racial justice is strongly valued for this peer/community health worker role.
- Established trust or relationships within communities disproportionately impacted by HIV in Memphis/Shelby County.
- Harm reduction competency and experience using trauma-informed, culturally responsive, LGBTQI+/trans-affirming, and reproductive/sexual justice approaches.
- Experience with HIV/STI testing, motivational interviewing, peer navigation, benefits/coverage navigation, or community-based outreach.
- Bilingual or multilingual proficiency is a plus
CONFIDENTIALITY, ETHICS & SCOPE OF PRACTICE
This is a non-clinical community health worker position. The Community Care Connector does not diagnose medical conditions, prescribe medications, or provide services outside approved training and scope. The Connector must follow SisterReach confidentiality, privacy, data security, mandatory reporting, safe-contact, documentation, and professional-boundary policies, as well as applicable grant and partner requirements.

This position includes office-based, mobile, and community-based work. Duties may require local travel, community event setup, extended periods of walking, sitting or standing, and carrying routine outreach/testing materials.
