We are seeking an Occupational Therapist (OT) to join the rehabilitation team at Autumn Lake at Glade Valley. This full-time position offers the opportunity to provide individualized therapy while helping residents improve daily function, safety, and independence.

• Evaluate residents and develop individualized occupational therapy plans of care

• Provide skilled treatment addressing activities of daily living (ADLs), strength, coordination, balance, and functional performance

• Establish treatment goals and track resident progress

• Recommend adaptive equipment and techniques to support greater independence

• Adjust treatment plans based on resident needs and response to therapy

• Educate residents and caregivers on strategies for safe daily activities

• Maintain accurate and timely clinical documentation

• Collaborate with COTAs, nursing staff, and other rehabilitation professionals

• $43–$50/hour

• Graduate of an accredited Occupational Therapy program

• Strong clinical, communication, and teamwork skills

• Skilled nursing or rehabilitation experience preferred, but not required
