Shift & Schedule: Day Shift | 8:00 AM – 4:30 PM | Monday – Friday | 5×8 Hours

Position Overview
We are seeking an experienced Medical Social Worker to provide psychosocial support, discharge planning, and care coordination for patients in an inpatient acute-care setting.
Required Qualifications
Master’s Degree in Social Work (MSW) required.
Minimum 2 years of inpatient acute-care experience.
Epic EMR experience required.
Strong discharge planning and interdisciplinary collaboration skills.
Ability to participate in one weekend day per month rotation.
Responsibilities
Complete comprehensive psychosocial assessments for hospitalized patients.
Develop and coordinate safe discharge plans.
Provide crisis intervention, counseling, and emotional support.
Connect patients and families with community resources and post-acute services.
Collaborate with physicians, nurses, and case management teams.
Document patient care accurately in Epic EMR.
Support patient care coordination and continuity throughout the hospital stay.
