Job Summary
Physical Therapist Home Health in a home health setting. This role involves providing outpatient physical therapy services to patients in their homes, focusing on patient rehabilitation and mobility. The position offers a short-term assignment with flexible training and orientation.
Job Details
- Certifications and Requirements: BLS certification, New Jersey Physical Therapy License, valid Driver's License, and Auto Insurance. Minimum 1 year of experience required.
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM, 40 hours per week.
- Responsibilities: Conduct regular visits, including start of care, recertification, and resumption of care. Typical workload involves 5-6 visits per day, with the possibility of 3 additional visits if performing an admission assessment.
- Additional Information: Candidates should preferably have prior home health experience and OASIS training. The role may involve performing safety assessments and patient education during visits. Short orientation with a preceptor for 1-2 weeks will be provided.
