Care Coordination Manager

Overview
The Care Coordination Manager plays a crucial role in overseeing and coordinating care for our clients. This position is responsible for ensuring that clients receive the appropriate services and support. The Care Coordination Manager will work closely with clients, families, and multidisciplinary teams to help develop and implement care.

Duties

• Supervise and support staff in delivering high-quality care and services.
• Maintain effective relationships with clients, families, and external service providers to ensure comprehensive care.
• Assist with team meetings to discuss needs, program development, and service delivery.
• Provide administrative support including documentation, reporting, and compliance.

Skills

• Excellent relationship management abilities to foster trust and collaboration among clients, families, and team members.
• Proficient in administrative tasks including documentation and compliance management.
• Experience in supervising staff to ensure high standards of care are maintained.
• Proven ability to manage multiple priorities while maintaining attention to detail.
• Background in social work or related fields with knowledge of behavioral health practices preferred
