\nRN - Admissions/Transfers
\nJob Details:
\n• 13-week contract with 36 guaranteed hours per week
\n • 12-hour shifts with variable days or nights; start times at 7:00 AM, 11:00 AM, 1:00 PM, and 7:00 PM
\n • Every other weekend and every other holiday required
\n • Can be placed on low census/on call; call back minimum is 2 hours
\n • Facilitate consults and transfers for patient placement across 16 hospital locations
\n • Coordinate patient admissions from other healthcare facilities, physician offices, and clinics
\n • Follow EMTALA protocols, policies, and algorithms while demonstrating system and regulatory understanding
\n • Anticipate admission needs and communicate findings with transferring departments to optimize patient flow
\n • Coordinate patient transfers across all disciplines including disposition, ambulance transportation, and specialist referrals
\n • Provide estimated time of arrival and pertinent handoff information to referring and accepting sites
\n • Accurately document and track all patient transfers into and out of the system
\n • Facilitate patient movement for leave of absence procedures or testing within the system
\n • Collaborate closely with bed planning teams to efficiently accept critically ill patients
\n • Use critical thinking skills to ensure placement of the right patient in the right bed
\n • Remotely engage with facility staff to monitor, document, and communicate patient condition and transfer needs
\n • Utilize chain of command to advocate for priority admissions or transfers to a higher level of care
\n • Plan, implement, and evaluate patient needs based on reported assessments to optimize outcomes
\n • Remotely triage using critical care or emergency department experience to identify patient care needs
\n • 2 RNs working nights; 2-4 RNs working days
\n • Scrub colors: any color or business casual
\n • Required certifications: WI RN License, AHA or ARC BLS
\n • EPIC charting experience required
\n • Minimum 3-5 years of ICU or ER experience required; 5 years preferred
\n • Trauma Level II experience required
\n • Transfer Call Center experience highly preferred
\n • Associate Degree or Certificate required
