Join our team as a Case Manager Registered Nurse in Eureka, CA! This is an exciting opportunity for an experienced RN to make a difference in patient care while enjoying the beautiful surroundings of Northern California.

As a Case Manager RN, you will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their healthcare journey. We are looking for candidates with at least 2 years of acute care experience. First-timers are accepted with clinical approval.

Key Responsibilities:

• Coordinate patient care and case management activities.
\n
• Work collaboratively with healthcare teams to ensure quality care.
\n
• Assist patients in navigating the healthcare system.
\n
• Document patient interactions and care plans accurately.
\n

Requirements:

• Must have acute care experience.
\n
• Required to work every other weekend.
\n
• Ability to float between like units as needed.
\n
• All RTO (Requests Time Off) must be included at the time of submittal.
\n

\n

Charting will be done using Epic, and free parking will be provided on the first day, with assignment parking given during orientation.

\n
