Responsibilities:

• Provide high-quality care to patients in a post-acute rehabilitation setting, under the supervision of licensed nursing staff

• Assist with daily living activities, such as bathing, dressing, and grooming

• Monitor and report changes in patient condition to licensed nursing staff

• Maintain accurate and confidential records of patient care

• Collaborate with interdisciplinary team members to develop and implement individualized care plans

• Provide emotional support and companionship to patients and their families
