Part-Time Home Health Admissions RN

ilver Haven Home Health LLC is seeking an experienced Part-Time Admissions Registered Nurse (RN) to complete patient admissions and Start of Care visits throughout the Dallas area. The ideal candidate has home health experience, strong assessment skills, and knowledge of OASIS documentation. This is a flexible position with hours based on patient admissions and agency needs.

Duties

• Complete home health admissions and Start of Care (SOC) visits.
• Perform comprehensive nursing assessments and OASIS documentation.
• Develop and initiate individualized plans of care.
• Complete medication reconciliation and patient education.
• Coordinate care with physicians, the DON, and interdisciplinary team.
• Ensure all admission documentation is accurate and submitted timely.
• Communicate changes in patient condition or identified care needs.

Requirements

• Current and unrestricted Texas RN license.
• Home health experience required.
• OASIS experience strongly preferred.
• Strong assessment and documentation skills.
• Current CPR/BLS certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Ability to travel to patient homes throughout the Dallas service area.
• Ability to work independently and manage documentation efficiently.
