Position Overview
Cherish Hospice is seeking a dedicated PRN Social Worker to join our compassionate team in Springfield, Ohio. This vital role provides flexible, as-needed psychosocial support to patients, families, and caregivers during one of life's most profound transitions.
Key Responsibilities
Conduct psychosocial assessments within 5 days of admission, identifying emotional, social, financial, and environmental needs.
Participate in IDG meetings and collaborate with the interdisciplinary team to deliver whole-person care.
Observe, record, and report changes in the patient's emotional and social well-being throughout the course of care.
Assess the patient's and family's ability to cope with the dying process, offering meaningful support every step of the way.
Evaluate the effectiveness of social work interventions and adjust approaches to meet evolving needs.
Participate in discharge planning as needed.

Required Qualifications
Master's in Social Work (MSW), licensed in the State of Ohio
Minimum 1 year of experience, preferably in hospice or palliative care
Strong interpersonal, communication, and crisis intervention skills

Deep commitment to compassionate, patient- and family-centered care
