Job Description

 The Health Department Registered Nurse (RN) provides community-focused care by assessing health needs, developing and implementing disease prevention programs, and providing health education to individuals and groups. Key responsibilities include conducting health assessments, administering immunizations, educating communities on healthy lifestyles, coordinating access to care, analyzing health trends, and collaborating with other health professionals and agencies to improve overall public health outcomes.  

Core Responsibilities

  • Direct Patient & Community Care: 
    Provides direct nursing services, including health assessments, treatments, and counseling, to individuals, families, and various population groups. 
  • Health Education & Promotion: 
    Develops and delivers health education programs and campaigns to prevent diseases and promote healthy lifestyles. 
  • Disease Prevention & Control: 
    Manages and implements programs such as immunization clinics, screenings, and other measures to control and prevent the spread of diseases. 
  • Community Resource Linkage: 
    Connects individuals and families with necessary health services and coordinates care for those who might otherwise be unable to access it. 
  • Health Data Analysis & Monitoring: 
    Monitors community health trends and analyzes health data to identify risk factors and needs. 
  • Collaboration & Advocacy: 
    Works with community partners, other healthcare professionals, and health departments to advocate for health-promoting policies and initiatives. 

Key Activities 

  • Conducting physical examinations and health screenings.
  • Administering vaccines and other medications.
  • Providing wound care and changing dressings.
  • Developing and managing patient and family care plans.
  • Tracking and maintaining patient medical records.
  • Following up with patients to monitor progress and ensure continuity of care.
  • Assessing environmental and social factors impacting community health.
