Job Description
Responsibilities:
• Guide individuals with disabilities and their families through federal and state work incentive programs, helping them understand how employment may affect public benefits and health coverage.
• Meet with clients to complete intake interviews, assess service needs, maintain organized case records, and document research and follow-up activities accurately.
• Build relationships with government offices, nonprofit organizations, employment networks, and other community partners that support people with disabilities.
• Lead outreach activities, informational sessions, and community presentations to increase awareness of available services and work incentive opportunities.
• Evaluate employer-sponsored health coverage and discuss alternative benefit options that may be available to clients based on their circumstances.
• Provide individualized work incentive planning that supports beneficiaries as they pursue employment goals and navigate benefit changes.
• Create and update educational resources that make complex benefit and work incentive topics easier for clients and families to understand.
• Prepare quarterly reporting that highlights program outcomes, service activity, and progress toward contract deliverables.
• Deliver training and technical guidance to community members and human service professionals regarding disability-related work incentives and referral pathways.
• Refer beneficiaries to suitable employment networks and coordinate with specialized work incentive contacts and field office resources as needed.
• Associate degree required.
• At least 2 years of experience in benefits coordination, disability services, case management, or a related field.
• Working knowledge of federal, state, and local disability benefit and work incentive programs.
• Experience conducting outreach, public education, or training for clients, families, or community-based organizations.
• Ability to manage case files, complete assessments, maintain documentation, and prepare outcome-based reports.
• Proficiency with Microsoft Office and database systems used for tracking services and client information.
• Strong communication and interpersonal skills, with the ability to explain complex benefit information clearly and respectfully.
• Familiarity with disability benefits, claims processes, leave-related matters, and compensation or benefits administration is preferred.
