✅ About the FacilityAhava Therapy Services is seeking a PRN OT to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility offering short- and long-term rehabilitation.

Review clinical information and evaluate its impact on the assessment and treatment process.
Conduct comprehensive functional assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable functional goals.
Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
Revise treatment plans based on patient performance and progress.
Supervise and guide therapy assistants, ensuring proper treatment techniques and care delivery.
Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
Assist with discharge planning and stay current on community resources.
Participate in care conferences, meetings, and quality improvement initiatives.
Maintain clinical competence through continuing education opportunities.
Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
Support the company’s mission, vision, and professional expectations.
Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
Bachelor’s or Master’s Degree in Occupational Therapy
Active OT license in the state of practice
BLS/CPR certification (current or obtained during orientation)
Strong clinical reasoning, communication, and documentation skills
Ability to collaborate effectively within an interdisciplinary team
