Local: $33.00 - $36.00/hour

Position Overview

We are seeking an experienced LPN/LVN to provide nursing care to residents in a Long-Term Care setting.

Required Qualifications

• Active New York State LPN License

• Current BLS Certification

• Minimum 1 year of LTC experience

• Recent Long-Term Care facility experience

Responsibilities

• Provide direct nursing care to LTC residents.

• Assess residents and monitor changes in condition.

• Administer medications and treatments according to orders and facility protocols.

• Assist with individualized care plans.

• Document patient care, assessments, medications, and interventions in PointClickCare EMR.

• Care for residents with MS, orthopedic conditions, dementia, and memory-care needs.

• Communicate changes in resident condition to the healthcare team.

• Collaborate with CNAs, RNs, physicians, and other healthcare professionals.

• Follow infection-control, safety, and facility policies.

• Maintain accurate clinical documentation and regulatory compliance.

• Float within the same building as needed.

• Work every other weekend.

• Perform other LPN/LVN duties as assigned.
