Physical Therapist Job Summary
We are seeking a dedicated Physical Therapist to provide professional physical therapy services to patients in their own homes. This role is essential in assisting individuals with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients served may vary in age from newborns to seniors over 100 years old.
Key Responsibilities- Deliver physical therapy services to patients in their residences.
- Collaborate with patients, families, physicians, and other health professionals to assess patient needs and coordinate appropriate services.
- Utilize thorough and timely electronic documentation in accordance with agency policies and standards.
- Communicate effectively with interdisciplinary teams to ensure continuity of care.
- Manage a caseload of patients, providing high-quality care and support tailored to individual needs.
- Participate in weekend rotations every six weeks.
- Travel to assigned cases as necessary.
- Must possess a valid Physical Therapist professional license in Massachusetts.
- Minimum of 1 year of experience in home care is required.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
