Territory: Marysville, Plain City, London, Jefferson & surrounding communities
Schedule: Monday–Friday, 8:00 AM–4:30 PM | Holiday & Weekend Rotation | Field-Based
Are you an RN who wants more time to care for patients and less time feeling rushed?
As a Hospice RN Care Manager, you'll provide skilled, compassionate nursing care to patients in their homes and care facilities throughout Marysville and surrounding communities.
This is an opportunity to use your clinical judgment, build meaningful relationships with patients and families, and work independently while having the support of a collaborative interdisciplinary team.
No hospice experience? That's okay. If you're an experienced RN ready for a different kind of nursing career, we'll provide the support and training you need to learn hospice.
What You'll DoProvide compassionate, patient-centered nursing care in homes and care facilities.
Assess patients and families and develop individualized plans of care.
Monitor changes in condition and adjust medications, interventions, care plans, and visit frequency as needed.
Communicate patient changes and care needs with physicians and the hospice team.
Coordinate care with nurses, physicians, social workers, aides, chaplains, volunteers, and other interdisciplinary team members.
Educate and support patients, families, and caregivers.
Advocate for patients and help families navigate the hospice experience.
Complete accurate and timely documentation in the electronic health record.
Participate in hospice admissions, interdisciplinary team meetings, care conferences, and quality improvement activities.
Support and mentor team members as needed.
Graduate of an accredited nursing program.
Current RN license in the State of Ohio in good standing.
Current CPR certification.
Valid driver's license and safe driving record.
Required automobile insurance.
Ability to travel throughout the assigned territory and safely drive in varying weather conditions.
Ability to successfully complete required background check and drug screening.
Basic computer skills and accurate documentation ability.
Two years of acute care nursing experience.
Hospice or palliative care experience.
CHPN certification.
New to hospice? No problem. Certification preparation and support are available for eligible employees.
