Summary of Position
As a CRA, you will serve as a trusted resource, helping individuals and families navigate healthcare programs, connect to community services, and overcome barriers to wellness. You will build meaningful relationships, provide education and enrollment assistance, and represent Kintegra Health at community events. Moreover, the CRA will provide patients and the public with assistance in meeting their healthcare needs through the following activities: Outreach/in reach, social health assessments, health insurance and safety net application assistance which include ACA Health Insurance Marketplace, Medicaid ePASS, Seniors’ Health Insurance Information Program (SHIIP), Medication Assistance Program and Sliding Scale enrollment. You will execute public education sessions, support and coordinate NHCW activities, participate in health center call to action campaigns, develop and strengthen relationships with stakeholders, adhere to all program guidelines, maintain SHIIP-STARS/CMS Navigator or CAC certification, and document accurately and timely across all platforms.
Minimum Qualifications
• Preferred experience in healthcare, community outreach, social services, or customer service.
• Strong communication, organizational, and problem-solving skills.
• Bilingual (English/Spanish) preferred.
• Commitment to serving others with compassion, professionalism, and respect.
Education: High School Diploma or GED certificate required.
Certification required following onboarding: Navigator or Certified Application Counselor, and SHIIP.
Key Responsibilities
• Maintain confidentiality.
• Connect patients and community members to healthcare and support services.
• Assist with applications and enrollment for programs such as Medicaid, ACA Marketplace plans, Sliding Fee Scale, Medication Assistance Programs, and SHIIP.
• Conduct social health assessments using motivational interviewing techniques.
• Provide health education, outreach, and community presentations.
• Build and maintain positive relationships with staff, community organizations and stakeholders.
• Participate in health fairs, outreach events, National Health Center Week activities, and advocacy initiatives.
• Document services and referrals accurately in required systems.
• Support quality improvement efforts and maintain compliance with program requirements.
• Collaborate with staff to maintain efficient workflow.
• Utilize all data systems to process referrals and expedite patient connection to appropriate services.
• Adhere to all state, federal, local, community partner program guidelines, policies and procedures.
• Participate in applicable Point in Time Count (PiT) sponsored by the Continuum of Care (CoC).
• Perform other duties and responsibilities as assigned to support departmental and organizational objectives.
