Job Description

We are seeking a dedicated Physical Therapist (PT) to provide essential therapeutic services aimed at treating illness and injury through physical exercises and therapies. In this role, you will assist in the rehabilitation and restoration of bodily functions to enhance client wellness and independence. You will develop and implement therapy programs for patients of all ages with developmental deficits, physical injuries, or illnesses.

Responsibilities

• Coordinate PT services by delivering direct therapeutic care and teaching rehabilitation skills that promote client independence and safety.
• Design and adjust a plan of care based on initial assessment, continuous reassessment of client needs, medical history, diagnoses, physician orders, and client goals, including discharge plans when necessary.
• Apply treatments using physical agents such as heat, water, and electricity, utilizing equipment like whirlpool baths, electrical muscle stimulation, and ultrasound.
• Adhere to utilization guidelines and protocols.
• Formulate and implement appropriate teaching plans and communicate significant clinical findings to physicians and other involved personnel.
• Document all skilled and medically necessary care accurately and in a timely manner, ensuring compliance with Medicare, State, Federal, JCAHO, and insurance requirements.
• Maintain client confidentiality and participate in quality management programs to uphold standards and improve skills.
• Conduct supervisory visits for Physical Therapy Assistants and Home Health Aides, providing updated instructions as required.
• Participate in staff meetings, case conferences, and customer service initiatives to align with community goals and enhance service delivery.
• Exhibit leadership competencies through effective communication, relationship building, and teamwork.
