Key Responsibilities- Provide physical therapy services across multiple care settings including outpatient, inpatient, swing-bed, and home health
- Evaluate patients and develop individualized treatment plans to address functional deficits and rehabilitation goals
- Deliver evidence-based therapeutic interventions to promote recovery and improve patient outcomes
- Document patient progress, treatment plans, and clinical notes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care across settings
- Active Physical Therapist (PT) license in the state of New Mexico or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Ability to work effectively across outpatient, inpatient, swing-bed, and home health settings
- Expected 40 hours per week availability
- Prior experience in a hospital-based or multi-setting therapy environment
- Experience with swing-bed and home health patient populations
- Strong documentation and time-management skills
