Key Responsibilities
  • Coordinate and deliver direct physical therapy services to patients of all ages, promoting maximum independence, functionality, and safety through individualized therapeutic care and patient/caregiver education.
  • Develop, implement, and revise individualized plans of care based on initial and ongoing assessments, physician orders, client history, diagnosis, expected outcomes, and discharge planning needs.
  • Perform and monitor the application of physical therapy treatments including the use of physical agents such as heat, water, and electricity utilizing equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound.
  • Document all skilled and medically necessary care thoroughly, accurately, and in a timely manner in compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements.
  • Supervise Physical Therapy Assistants and Home Health Aides in accordance with applicable Federal and State laws, regulations, and agency policy; conduct supervisory visits and maintain written instructions as applicable.
  • Participate in staff meetings, case conferences, quality management programs, and required in-service training to maintain current skills and support facility goals.
Required Qualifications
  • Active Physical Therapist (PT) license in good standing, or eligibility to practice in the state of South Dakota.
  • Ability to perform all duties within the scope of physical therapy practice guidelines and in accordance with productivity standards.
  • Knowledge of and ability to comply with Medicare, State, Federal, JCAHO, and payer documentation and care requirements.
  • Strong communication and relationship-building skills with patients, caregivers, physicians, and interdisciplinary team members.
Preferred Experience / Skills
  • Experience working with patients across a variety of age groups and diagnoses including developmental deficits, physical injury, and illness.
  • Prior experience in a home health or outpatient setting with familiarity supervising PTAs and Home Health Aides.
  • Experience with quality improvement programs and utilization guidelines.
