Position Title: Care Manager (RN)
Work Location: Florida-Remote
Assignment Duration: 6 Months (possibility to extend or convert)
Work Arrangement: Remote

Position Summary:
Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

Background & Context:
Case Management-Medicaid. Backfill. Very Engaged Team environment, work is fast paced.

Key Responsibilities:
* Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families
* There are monthly metrics that all CM's are required to meet including but not limited to expected cases loads
* What makes this role unique is that it is a remote position that deals directly with members

Education/Certification Required: Degree from an accredited School of Nursing or a Bachelor's degree in Nursing.
Preferred: Licensure Required: RN Preferred: Years of experience required: Requires 2 to 4 years of related experience.
