Position Title: Care Manager (RN)
Work Location: Remote- FL
Assignment Duration: 3 months (Possible to extend or Convert)
Work Schedule: Work from home 8 am-5pm
Work Arrangement: Work from home
Position Summary:
* Enhanced Discharge Planning Team (EDCP) who make outbound calls to Medicaid plan members who are discharged from the hospital to prevent re-admissions.
* EDCP team assesses immediate needs/barriers, complete transition of care assessment by day 10 post discharge day and attempt to remove barriers and meet needs and follow up post discharge appt with a medical or BH provider.

Qualification & Experience:
* Education/Certification: Required Associate or Baccalaureate degree in nursing Preferred: Baccalaureate degree in nursing
* Licensure: RN (compact is okay) Preferred: RN
* Must haves: 2-3 years Care Management experience with all age groups
* Nice to haves: Discharge planning experience; Care Management Certification
* Disqualifiers: Cannot answer questions regarding your past work experience during interview.
* Performance indicators: Audit scores 90% or above monthly; 1:1 meetings with Supervisor
* Top 3 must-have hard skills: 1 Must be good with technology 2 Outbound calling experience 3 Critical thinking
* 4 Must be able to work in fast paced environment; quick learner; hit the ground running for this temp role
