Home Health RN – 13-Week Travel Contract

KPG Healthcare is seeking an experienced Home Health RN for a full-time 13-week travel assignment. The RN will provide skilled nursing care to patients in their homes, completing scheduled visits and managing care based on each patient's individual needs.

Responsibilities:

• Provide skilled nursing care during in-home patient visits
• Assess patients and monitor changes in condition
• Develop and implement individualized plans of care
• Perform wound care, medication management, and other skilled nursing services as needed
• Complete OASIS assessments and required documentation
• Coordinate care with physicians, therapists, and other members of the healthcare team
• Document visits using Homecare Homebase (HCHB)

Requirements:

• Active RN license in the state of practice
• 1+ year of recent Home Health experience
• OASIS experience required
• BLS certification (AHA)
• Homecare Homebase (HCHB) experience preferred
• Associate's degree or higher
