Position Overview

We are seeking a Physical Therapist (PT) for a 13-week Home Health contract assignment in San Antonio, TX.

The Physical Therapist will provide skilled therapy services to patients in their homes throughout the assigned service area. The position offers a consistent Monday–Friday schedule with 40 hours guaranteed, no weekends, and no on-call responsibilities.

Key Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes
• Develop individualized treatment plans based on patient needs and goals
• Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence
• Monitor patient progress and modify treatment plans as appropriate
• Complete timely and accurate clinical documentation in HCHB
• Coordinate care with physicians, nurses, therapists, and other members of the healthcare team
• Educate patients and caregivers on appropriate exercises, safety, mobility, and home programs
• Oversee and supervise PTA activities in accordance with applicable requirements
• Manage an assigned caseload while maintaining productivity and quality-of-care standards
