Addison Group is partnering with our client to hire an HCC Practice Performance Manager to support a growing healthcare organization in the Wichita market. This is a highly visible, provider-facing position focused on strengthening physician relationships, improving HCC and risk adjustment performance, and supporting value-based care initiatives across primary care practices.
This position is ideal for someone who enjoys working directly with providers, analyzing performance data, and taking ownership of results in an autonomous environment.
Job Description:
The HCC Practice Performance Manager will serve as a key point of contact for physician practices throughout the Wichita market. This individual will partner with providers and practice leadership to evaluate performance, identify documentation and risk adjustment opportunities, and implement strategies designed to improve overall results.
The position combines provider relationship management, HCC/risk adjustment knowledge, data analysis, education, and value-based care strategy. Candidates should be comfortable managing their own schedule, traveling between practices, and operating independently.
Key Responsibilities:
- Develop and maintain strong relationships with physicians, clinic managers, and practice leadership.
- Partner with providers to improve HCC capture, clinical documentation, coding accuracy, and overall risk adjustment performance.
- Review provider and practice-level data to identify trends, performance gaps, and areas requiring additional attention.
- Conduct onsite meetings with providers and practice teams to review findings and establish actionable improvement plans.
- Educate providers on documentation practices that support accurate representation of patient conditions and acuity.
- Monitor HCC, quality, and value-based care performance and use data to prioritize provider outreach.
- Help physician practices understand performance opportunities and implement solutions that support measurable improvement.
- Maintain consistent communication and follow-up with assigned practices to track progress and drive accountability.
- Collaborate with internal stakeholders on broader population health, primary care, and value-based care initiatives.
- Independently manage provider meetings, reporting, market travel, and follow-up responsibilities.
Qualifications:
- Previous experience working directly with physicians, medical practices, or healthcare leadership.
- HCC, Risk Adjustment, Value-Based Care, Population Health, or similar healthcare performance experience preferred.
- CPC or CRC certification strongly preferred, although candidates with strong provider-facing healthcare backgrounds who are interested in expanding their coding expertise may be considered.
- Experience in provider relations, nursing, healthcare operations, population health, medical/pharmaceutical sales, coding, or similar provider-facing environments may be considered.
- Strong analytical skills with the ability to interpret performance data and communicate actionable findings.
- Proficiency with Excel and comfort working with healthcare performance metrics and reporting.
- Excellent communication, presentation, and relationship-management skills.
- Ability to operate independently with limited day-to-day supervision.
- Strong organizational skills, follow-through, and personal accountability.
- Must be comfortable with frequent local travel throughout the assigned market.
Additional Details:
- Monday-Friday schedule, generally 8:00 AM-4:00 PM or 9:00 AM-5:00 PM, aligned with provider schedules.
- Approximately 75% local travel to physician practices throughout the Wichita market.
- Most assigned locations are within approximately 30 minutes of Wichita.
- Individual contributor position with significant interaction with providers and healthcare leadership.
- High level of independence and ownership over assigned practices and outcomes.
