Job Summary:

Washington Square Nursing & Rehab, a 83-bed skilled nursing facility located in the heart of Warren, OH, is seeking a compassionate and dedicated Registered Nurse to join our team. As a key member of our healthcare staff, you will play a vital role in providing exceptional care to our residents. Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions

• Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals

• Participate in multidisciplinary team meetings to discuss patient progress and develop strategies for improvement

• Maintain accurate and up-to-date patient records, including assessments, progress notes, and discharge summaries

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination

• Provide emotional support and education to patients and families, addressing their physical, emotional, and psychological needs
