Shared Position
This role involves a hybrid schedule that includes three days at one location and two days at another location within the region. The position requires collaboration across both settings to deliver quality services and support.
Responsibilities- Provide essential services and support to clients and residents.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
- Maintain accurate and timely documentation of services provided.
- Participate in team meetings and contribute to care planning.
- Adapt to changing needs and priorities of the locations served.
- Relevant experience in the field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Flexibility to travel between two locations as required.
- Professional licensure or certification as applicable to the role.
