About the Role

Anuco Home Health Care LLC is seeking an experienced, highly organized Home Health Intake Coordinator to manage referrals from initial receipt through admission readiness.

This is a high-impact position responsible for ensuring that patient information, insurance benefits, authorizations, physician documentation, orders, and clinical records are accurate and complete before services begin. The Intake Coordinator serves as a central point of communication among patients, families, referral sources, physician offices, insurance companies, clinicians, and internal departments.

The ideal candidate understands Medicare-certified home health operations, works confidently in Axxess Home Health, and can manage a high-volume referral pipeline without allowing important details or deadlines to be missed.

Key Responsibilities

  • Manage home health referrals from receipt through acceptance, scheduling, and admission handoff.

  • Review referral packets for demographics, diagnoses, clinical records, medication information, physician information, face-to-face documentation, and home health orders.

  • Confirm patient name, date of birth, address, telephone numbers, emergency contacts, insurance coverage, primary physician, last physician visit, and upcoming appointments.

  • Verify insurance eligibility, benefits, network status, deductibles, copayments, coinsurance, and payer-specific admission requirements.

  • Determine whether prior authorization is required and initiate requests before the start of care whenever possible.

  • Track approved disciplines, visits, units, authorization numbers, effective dates, and expiration dates.

  • Obtain missing physician orders, face-to-face documentation, hospital records, discharge summaries, and other required information.

  • Enter and maintain accurate patient, referral, insurance, physician, and admission information in Axxess.

  • Upload, label, and file documents in the correct sections of the electronic patient record.

  • Maintain a real-time record of missing information, pending authorizations, outstanding orders, admission barriers, and completed follow-up.

  • Communicate professionally with patients, families, hospitals, physician offices, referral sources, payers, clinicians, and internal staff.

  • Coordinate with scheduling and clinical leadership to confirm staffing availability and start-of-care readiness.

  • Provide complete handoffs to scheduling, clinical, authorization, orders, QA, and billing personnel.

  • Monitor referrals for timely acceptance, delay, or denial and communicate status updates to referral sources.

  • Send, receive, and route documentation using SRFax and other approved electronic systems.

  • Identify discrepancies or compliance concerns and escalate them promptly to agency leadership.

  • Protect confidential information and comply with HIPAA, Medicare requirements, payer rules, and agency policies.

Axxess Responsibilities

  • Demonstrate strong proficiency with Axxess Home Health.

  • Navigate referrals, patient charts, eligibility information, insurance records, physicians, documents, orders, schedules, notes, tasks, and reports.

  • Enter referral and admission information accurately and consistently.

  • Create and monitor tasks for missing documentation and outstanding requirements.

  • Confirm that referral documents are uploaded to the correct patient and chart section.

  • Identify workflow issues that could delay admission, authorization, billing, or patient care.

  • Help staff resolve non-clinical Axxess workflow and documentation concerns.

Required Qualifications

  • At least two years of home health intake, referral coordination, insurance verification, authorization, or comparable healthcare operations experience.

  • Experience working for a Medicare-certified home health agency.

  • Hands-on proficiency with Axxess Home Health or a comparable home health EMR.

  • Working knowledge of Medicare, Medicare Advantage, Medicaid, and commercial insurance requirements.

  • Understanding of home health orders, face-to-face documentation, Plans of Care, physician information, and admission requirements.

  • Strong insurance verification and prior-authorization skills.

  • Excellent written and verbal communication skills.

  • Professional telephone etiquette and customer-service skills.

  • Exceptional attention to detail, follow-through, organization, and time management.

  • Ability to manage multiple referrals and deadlines in a fast-paced environment.

  • High school diploma or GED.

Preferred Qualifications

  • Three or more years of intake experience with a Medicare-certified home health agency.

  • Advanced proficiency with Axxess Home Health.

  • Experience using CarePort, Availity, HHAeXchange, SRFax, Microsoft 365, and payer authorization portals.

  • Knowledge of Medicare face-to-face requirements, physician certification, NOA readiness, and common admission-related denial risks.

  • Experience coordinating skilled nursing, therapy, medical social work, and home health aide referrals.

  • Associate degree or higher in healthcare administration, health information management, business, or a related field.

  • Bilingual communication skills are a plus.
