360X Staffing is seeking a Field Care Coordinator to help members and families navigate healthcare services, connect with community resources, and overcome barriers to care. This opportunity is a strong fit for someone with experience in care coordination, health coaching, or community services who enjoys building relationships and helping people make progress toward their health goals.

Applicants must currently live in Pasco County, Florida. This is a patient-facing role.

What You’ll Do

• Assess members’ health-related needs and identify barriers to accessing care.
• Help members and families connect with appropriate healthcare, community, and social service resources.
• Support individualized care planning through assessments, member conversations, and available data.
• Monitor progress toward care goals and help address needs as they arise.
• Maintain accurate, timely case notes across electronic documentation systems.
• Follow established procedures, regulations, and professional standards.

Required Qualifications

• Bachelor’s degree.
• At least 2 years of related experience.
• Current residency in Pasco County, FL.
• Strong verbal and written communication skills and the ability to build rapport with members and families.
• Proficiency with Microsoft Word and Excel, plus the ability to work across multiple software applications.
• Valid state driver’s license and a personal vehicle.
• Auto liability insurance meeting 100/300/100 coverage limits.
• Willingness to complete TB screening.
• Commitment to improving member experiences and access to care.

Preferred Qualifications

• Master’s degree.
• Experience working with Medicare and Medicaid recipients.
• Experience with electronic case notes and documentation across multiple systems.
• Background in health promotion, coaching, or wellness.
• Knowledge of community health agencies, social services, and local resources.
