Travel Contract – Submission Requirements

Required

• 3+ years of current/recent experience — must have been worked within the last 6 months
• Must be willing to float to areas where the candidate has experience
• IV certified, or willing to obtain IV certification during onboarding
• All RTO (Requested Time Off) must be disclosed at the time of initial submission and approved in advance
• • RTO cannot exceed 5 days
  • Holiday RTO is not permitted
• COVID-19 vaccination required
• • Declinations/exemptions not accepted
  • Proof of vaccination must be uploaded with the profile
• Candidate must reside more than 50 miles from the facility
• • Permanent address must be clearly listed in the profile

Highly Preferred

• Previous travel assignment experience
• Experience working in a rural or small facility

Important: Candidates who do not meet the required experience, vaccination, distance, IV certification/willingness, floating, and RTO requirements may not be considered.
