Youth Community Enrichment Case Manager

Do you have a passion for helping children with intellectual and developmental disabilities (IDD) reach their full potential? The Arc Fond du Lac is seeking a compassionate and organized Youth Community Enrichment Case Manager to support youth participants and their families through our Youth Community Enrichment Program (YCEP).

Key Responsibilities

• Manage a caseload of youth with IDD and develop person-centered service plans.
• Coordinate participant admissions, assessments, annual reviews, and discharges.
• Work closely with families, guardians, schools, and service providers.
• Plan engaging activities, community outings, and skill-building opportunities.
• Ensure compliance with Wisconsin CLTS Waiver regulations and documentation requirements.
• Assist with staff training, onboarding, and program oversight.
• Support participant health, safety, and overall quality of care.
• Drive accessible vehicles for community-based activities as needed.

Qualifications

• Age 21 or older.
• Coursework in Advanced Early Childhood Development or at least 2 years of experience working directly with individuals with intellectual and developmental disabilities.
• Minimum 1 year of supervisory experience.
• Valid Wisconsin driver's license with a clean driving record.
• Strong communication, organization, and leadership skills.
• Proficiency with Microsoft Office.
• Ability to pass a background check and pre-employment drug screen.

Schedule & Benefits

• Full-time, exempt position
• Primarily weekday hours
• Required 1 Saturday per month during the school year
• Occasional evening/weekend hours as needed
