Physical Therapist Job Summary
We are seeking a skilled Physical Therapist to provide therapy services to patients in their homes, addressing physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Our patients range in age from birth to over 100 years and require personalized therapy plans to enhance their quality of life.
Responsibilities- Deliver high-quality physical therapy services to patients in their residence.
- Assess patient needs and develop individualized treatment plans.
- Collaborate with patients, families, physicians, and other healthcare professionals to coordinate services.
- Provide education and support to patients and their families regarding therapy and recovery.
- Maintain accurate and timely electronic documentation of patient progress and interventions.
- Participate in interdisciplinary communication in accordance with agency policies and standards.
- Must possess a valid Physical Therapist Professional license in Massachusetts.
- Minimum of 1 year of experience in home care settings is required.
- Ability to travel to various patient cases as assigned.
- Availability for weekend rotations approximately once every 6 weeks.
This role will serve patients across multiple locations within the South Region, specifically focusing on areas including Dorchester, Jamaica Plain, Mission Hill, and Roxbury.
