Job SummaryWe are seeking a dedicated Physical Therapist to provide essential therapy services to patients within their residences. This role involves working with individuals experiencing physical impairments, functional limitations, or disabilities due to various causes, including injury or disease. The patient demographic ranges from infants to elderly individuals over 100 years of age.
Responsibilities- Deliver high-quality physical therapy services tailored to each patient's individual needs.
- Collaborate effectively with patients, families, physicians, and allied health professionals to determine and coordinate appropriate care strategies.
- Maintain compliance with agency policies and standards of practice.
- Document therapy sessions promptly and accurately using electronic documentation systems.
- Travel to assigned cases as required.
- Must possess a valid Physical Therapist professional license in Massachusetts.
- Minimum of 1 year of experience in home care settings is required.
- Willingness to participate in weekend rotations once every six weeks.
The start date for this position will be determined based on the completion of credentialing processes. If clearance is not obtained by the expected start date, a rescheduled start may be necessary.
