Physical Therapist (PT) / Physical Therapist Assistant (PTA)

Key Responsibilities- Provide physical therapy or physical therapy assistant services to residents in a care facility setting
- Deliver skilled therapy interventions in alignment with each patient's individualized plan of care
- Collaborate with the interdisciplinary care team to support patient progress and goal attainment
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
- Present valid state or federal issued photo identification upon entry and wear a photo badge at all times while on site
- Active and unrestricted New Mexico PT or PTA license
- Valid state or federal issued photo identification required
- Ability to work Monday through Friday day shifts for the duration of a 13-week contract
- Prior experience in a skilled nursing or long-term care environment
- Familiarity with Medicare documentation and compliance requirements
- Expected 35 hours per week providing a consistent and stable schedule throughout the contract
