Travel SNF Physical Therapist – Heritage Manor Health Center & Guardian Angels Health & Rehab, Chisholm/Hibbing, MN
Position Summary
You'll treat short-term rehab and long-term care residents at two skilled nursing facilities on Minnesota's Iron Range on a full-time travel contract with Ardor.
This is a 13-week travel Skilled Nursing Facility (SNF) Physical Therapist role, with extension possible, supporting Heritage Manor Health Center in Chisholm and Guardian Angels Health & Rehabilitation Center in Hibbing. Both are faith-based care centers in the St. Francis Health Services family. You'll work days, five 8-hour shifts a week (40 hours), helping residents regain strength, mobility and independence after surgery, a hospital stay or illness. Heritage Manor is at 321 NE 6th Street, Chisholm, MN 55719, and offers specialized programs including vestibular, concussion and cognitive rehab. Guardian Angels, at 1500 3rd Avenue East, Hibbing, MN 55746, provides postoperative transitional care and long-term care. The two buildings are about 10 minutes apart.
New graduates and therapists without previous SNF experience are welcome to apply. You'll have support from the St. Francis rehab teams and Ardor's 24/7 clinical support while you build your skills.
