🏡 Travel Physical Therapist – 13 Week Contract | Days

We’re seeking a Physical Therapist (PT) for a Home Health travel assignment covering the Fremont & East Bay, CA areas!

📍 Territory: Fremont, Castro Valley, Hayward, Newark, San Leandro, Union City, Oakland, Berkeley & surrounding areas
⏳ Contract: 13 Weeks
☀️ Shift: Days
⏰ Guaranteed Hours: 36 hrs/week*
🚗 Travel: Patient-to-patient travel required | Weekend coverage may be required
🏥 Setting: Home Health

Requirements:

✅ Active California PT License
🩺 BLS required
🚗 Valid Driver’s License
🚙 Auto Insurance & Registration required
🧪 Annual TB, Flu & Drug Screen
💉 MMR, Varicella, Hep B & Tdap required
🩺 Physical exam required
📋 Minimum 3 years of PT experience

Assignment Details:

📊 27 points/week required to maintain guarantee*
🏡 May require visits outside the assigned area
🦠 Must be willing to see COVID+ / PUI patients
