Care Manager RN
A major healthcare company is seeking a Care Manager RN to support its Foster Care Care Management team serving children and youth in DCF custody. This is a remote, field-based position requiring significant travel throughout Northwest Kansas. The Care Manager will provide care coordination, assessments, care planning, member visits, and collaboration with families, schools, providers, DCF, and community organizations. The ideal candidate will bring RN clinical experience, behavioral health or child welfare experience, care management expertise, and familiarity with HCBS waiver programs.

Location: Remote, field-based — Northwest Kansas
Schedule: Monday–Friday, 8:00 AM–5:00 PM CST
Travel: Up to 75% throughout Northwest Kansas
Driver’s License: Required

Key Responsibilities

• Provide care management and coordination services to children and youth in foster care.
• Complete required 48-hour in-person home visits following a placement change.
• Conduct member visits based on acuity:
• • High acuity: Every other month
  • Moderate acuity: Every three months
  • Low acuity: Annually
• Complete Health Risk Assessments (HRA).
• Develop and maintain Plans of Service.
• Convene 7-day Interdisciplinary Team (IDT) meetings.
• Participate in Team Decision Making (TDM) meetings.
• Attend Community-Based Services Team meetings.
• Coordinate behavioral health, physical health, and social services.
• Collaborate with:
• • Department for Children and Families (DCF)
  • Care Management Providers (CMPs)
  • Community Mental Health and behavioral health providers
  • CCBHCs
  • Foster families
  • Schools
  • Healthcare providers
  • Community organizations
• Connect members with appropriate medical, behavioral health, social, and community resources.
• Monitor member needs and coordinate services based on acuity and individualized needs.
• Maintain accurate and timely documentation.
• Ensure care management activities meet contractual and regulatory requirements.
• Manage an assigned caseload and maintain required productivity and outreach standards.
• Work independently in the field while maintaining regular communication with the care management team.

Typical Day

The Care Manager will have a combination of member visits, assessments, care planning, team meetings, documentation, and provider/family coordination.

Once training is complete, Care Managers have significant autonomy over their schedules outside of standing meetings and can coordinate their member visits based on individual needs and geographic considerations.

Because this position serves a school-age population, occasional evening meetings may occur based on member and team needs.

Required Qualifications

• Bachelor’s degree OR Associate degree with appropriate certification/experience.
• Registered Nurse (RN) license required.
• Candidates with appropriate LMSW or LCSW credentials may also be considered.
• RN candidates should have behavioral health experience.
• Social work candidates should have at least 1–2 years of child welfare experience.
• Strong assessment and care management skills.
• Strong documentation and organizational skills.
• Ability to work independently in a remote, field-based environment.
• Ability and willingness to travel extensively throughout Northwest Kansas.
