Hiring a Medical Assistant Care Coordinator!

Job Description

We are seeking a dedicated and compassionate Medical Assistant Care Coordinator to join our team. This role supports our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, focusing on improving patient compliance, coordinating care, monitoring health metrics, and reducing unnecessary hospital and urgent care visits.

Responsibilities

• Conduct monthly CCM and RPM patient outreach calls.
• Review medications, treatment plans, and patient concerns.
• Monitor blood pressure readings and respond to RPM alerts.
• Coordinate care between patients, providers, pharmacies, and medical equipment vendors.
• Assist with obtaining and facilitating physician orders for durable medical equipment.
• Document patient interactions and maintain compliance within the CCM/RPM platform.
• Ensure Medicare-required patient engagement time is achieved and documented.
• Educate patients and provide support regarding chronic health conditions.
• Identify barriers to care and escalate concerns to providers when necessary.

• Ability to build rapport and comfortably converse with elderly and chronically ill patients

Additional Skills & Qualifications

• Prior CCM (Chronic Care Management) experience
• Prior RPM (Remote Patient Monitoring) experience
• Experience coordinating patient care, medication management, or follow-up care
• Experience in a physician practice, primary care, or care management setting

Work Environment

This position is office-based, working onsite Monday through Friday from 9:00 AM to 5:00 PM in Fresh Meadow, Queens. You will be part of a team currently consisting of three medical assistants, with plans to add two more. The role focuses on patient interactions and coordination over the phone.

This is a Permanent position based out of Fresh Meadows, NY.

The pay range for this position is $41600.00 - $52000.00/yr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.

W2 position offered paid sick time. No benefits offered other than sick time.

This is a fully onsite position in Fresh Meadows,NY.

This position is anticipated to close on Oct 20, 2026.
