About the Role:

As a CNA with Beechwood Post Acute & Transitional Care, you will be responsible for providing high-quality direct patient care to our residents, including assisting with daily living activities, monitoring vital signs, and reporting changes to the healthcare team. You will work closely with our nursing staff to develop and implement individualized care plans, and ensure that our residents receive the highest level of care and attention.

Responsibilities:

• Provide direct patient care to residents, including assisting with daily living activities, bathing, dressing, and grooming

• Monitor resident vital signs and report changes to the healthcare team

• Assist with the preparation of residents for meals and activities

• Maintain accurate and up-to-date records of resident care and services

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Participate in quality improvement initiatives to ensure high-quality care and service
