Social Services Designee

HARTVILLE CARE CENTER, located in Hartville, Missouri is currently seeking a Social Services Designee to join our team.

Social Services Designee Duties and Responsibilities:

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs

• Have a firm grasp of time management, prioritizing, and time sensitive documentation of patient charts.

• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, appointment scheduling, advanced directives.

• Promptly follows-up on admission and discharge needs for incoming and outgoing patients as well as paperwork needed. 

Director of Social Services Qualifications and Skills:

• Experience as a Director of Social Work in a large LTC setting preferred, but not required.

• At least one year experience in Social Work field preferred, but not required.
